medically untreated prolactinoma 28mm in size unilateral painful ptosis

before 1 week?Rx surgical/medical?? female 25 years having prolactinoma 28 mm in size with unilateral painful ptosis which is cleard after treated with manitol and antibiotics.not treated with bromocriptine or cabergoline .should i go for neurosurgical approach to remove prolactinoma or treat with cabergoline/???
